This Privacy Notice describes how Hombrex LLC. (“HOMBREX”) uses, discloses, and protects the personal information we collect through the HOMBREX website, JoinHOMBREX.com.

HOMBREX is not a medical provider and does not provide medical advice.

HOMBREX assists individuals in establishing a relationship with independent healthcare providers (“Healthcare Providers”).  We also provide certain non-medical services and information to users of our website.

New users are asked initial questions about themselves and their health to see if they might qualify to be referred to a Healthcare Provider. These screening questions were approved by the Healthcare Providers.  If the answers to those questions meet certain criteria specified by a Healthcare Provider, the Healthcare Provider will invite the user to schedule an initial appointment and establish an account with the Healthcare Provider through a patient portal (the “Patient Portal”),  The Patient Portal may have the HOMBREX logo, but it is operated by the Healthcare Provider or its service providers.

At the Patient Portal, the user will answer more detailed questions provided by the Healthcare Provide; additional medical information and other personal information will be requested.  

The user’s relationship with the Healthcare Provider, and the user’s use of the Patient Portal and other Healthcare Provider Sites, are subject to the privacy policies and practices of the Healthcare Provider and its service providers, not this Privacy Notice, even though the HOMBREX logo may appear on the Patient Portal.   

The user’s relationship with Third-Party Suppliers is subject to the privacy policies and practices of the Third-Party Suppliers and their service providers, not this Privacy Notice (regardless of whether the HOMBREX logo appears as part of the user’s interactions with them).

To Healthcare Providers, Healthcare Provider Sites, and Third-Party Service.  

We disclose information to companies that provide services to us, such as providers we may engage to host or operate our website, analyze data, or provide customer service, data storage, marketing, analytics, security, or fraud prevention.

In connection with legal matters: We may disclose information when we believe disclosure is appropriate due to a subpoena or similar investigative demand, a court order, or other request from a law enforcement or government agency, or as otherwise required by law.

We may disclose information when we believe disclosure is appropriate in connection with efforts to investigate, prevent, or take other action regarding illegal activity, suspected fraud or other wrongdoing; to protect and defend the rights, property or safety of our company, our employees, our customers, or others; and to enforce our contracts.

At HombreX.com, we are committed to safeguarding your Protected Health Information (PHI). We collect PHI only as necessary to provide our services and ensure compliance with applicable laws. The information collected may include your name, contact details, medical history, and payment information. We do not share your PHI with third parties unless required to provide our services or as mandated by law.
